COPIES OF BIRTH, DEATH, & MARRIAGE RECORDS
Birth Certificates:
- Birth records occurring in Mason County begin with 1867.
- Birth records are confidential records and are available only to the person or the parent named on the record.
- Additional documentation will be required of applicants stating they are an heir, legal guardian, or legal representative of the person named on the record.
- A current, valid driver's license or state issued ID is required for birth certificate requests in person.
- Requests for birth certificates by mail must include:
- ·Name
- ·Date of Birth
- ·Parent's Names (including mother's MAIDEN name)
- ·Notarized signature
- ·Self addressed stamped envelope
- ·Check or money order - payable to Mason County Clerk

Death Certificates:
- Death records occurring in MasonCounty begin with 1875.
- Death records are open to the public and are available for persons whose death occurred in MasonCounty.
- Requests for death records by mail must include:
- Name
- Date of Death
- Self addressed stamped envelope
- Signature
- Check or money order - payable to Mason County Clerk
- Fees are:
- Certified copies - $7.00 for the first copy and $3.00 for each additional copy of the same record.

Marriage Certificates:
· Marriage records on file in MasonCounty begin with 1867.
· Marriage records are open to the public and are available for licenses applied for and subsequently filed with the Mason County Clerk.
· Requests for marriage records by mail must include:
o Name of Groom
o Maiden name of Bride
o Date of Marriage
o Self addressed stamped envelope
o Signature
o Check or money order - payable to Mason County Clerk
